Health Requirements for Students
LSUHSC requires all medical, nursing, allied health, and dental students to meet the following health requirements for admission. All forms can be found in the "Forms/Records" tab above.
Enter your information into the AAMC Standardized Immunization Form and your student health portal.
- Varicella: Documentation of two (2) doses of Varivax vaccine OR a positive serologic IgG antibody titer confirming immunity.
- MMR (Measles, Mumps, Rubella): Documentation of a completed two (2) dose vaccine series OR positive serologic IgG antibody titers confirming immunity.
- Tdap: Documentation of one (1) dose administered within the past ten (10) years.
- Tuberculosis (TB):
- A negative TB blood test (QuantiFERON-TB Gold or T-SPOT) or a negative tuberculin skin test is required annually.
- Students who have received the BCG vaccine should receive a TB blood test due to the potential for false-positive skin test results.
- If a TB test is positive, submit proof of past treatment. If no treatment has been received, it is strongly recommended.
- Newly positive or past positive TB results require a chest X-ray and completed TB symptom review form prior to admission.
- Hepatitis B: Documentation of completion of the two (2) or three (3) dose vaccine series AND proof of immunity through a quantitative Hepatitis B surface antibody titer (≥ 10
mIU/mL).
- If antibody titers are below the protective threshold (<10 mIU/mL), a booster is required, followed by repeat titers in 4–8 weeks.
- Influenza Vaccine: One (1) dose required annually each fall.
- Meningococcal Vaccine: Highly recommended within the past ten (10) years for students residing in campus housing.
- COVID-19 Vaccine: Records may be entered for record-keeping purposes but are not mandatory.
If you are deficient in any of the above, schedule an appointment with your primary care physician or the LSUHSC Student Health Center for updated vaccines or titers.
- Submit the completed AAMC Standard Immunization Form along with supporting vaccine records and serologic lab titer documentation via the student health submission portal.
You can access the student health submission portal by visiting https://www.lsuhsc.edu and navigating: MENU → MyLSUHSC → Self Service → Academic Self-Service → log in with your LSUHSC username and password → My Forms → enter required information and upload documentation.
- When submitting documentation, attach records for all reported items, including medical records, lab results, and vaccine records.
- All documentation must include your full name (first and last) and date of birth.
Immunizations
TB AND FLU VACCINES ARE REQUIRED ANNUALLY!
This does not apply to Graduate, Public Health or Online Students.

Hours and Location
Monday - Friday 8:00 a.m. to 4:00 p.m.
Appointments preferred, but walk-ins are welcome.
